Case: Thrombus Aspiration with EnVast
A 55-year-old overweight man with STEMI and persistent angina for 24 hours was admitted to the catheterization lab. Coronary angiography documented diffuse coronary ectasia with thrombotic occlusion of the mid-right coronary artery.
• Failure of initial balloon angioplasty, IV tirofiban, and multiple attempts with aspiration.
• Thrombectomy with enVast 4.5 x 46 mm was performed with vacuum assistance, extracting a large thrombotic burden in 5 passes plus 2 additional passes.
• Rapid resolution of symptoms and complete resolution of ST-segment elevation.
• Absence of visible residual coronary stenosis after thrombectomy: decision made not to stent the vessel.
• Histological analysis of the thrombus recovered at each pass was examined: enVast recovered more complex and extensive thrombotic structures, ensuring improved thrombectomy.
