It was an anterior STEMI with thrombotic occlusion of the LAD. We performed an aspiration thrombectomy with a good yield but without restoration of antegrade flow. IVUS showed a high thrombus burden, so we decided to perform thrombectomy with the enVast stent retriever. After two attempts, antegrade flow was present, but there was still a lot of thrombus. After two more attempts, no thrombus was visible on IVUS. There were plaques and plaque rupture, but the minimum lumen area (MLA) was greater than 10 mm\u00b2. We used a 4 mm drug-coated balloon and did not place a stent.
